BEFORE booking an appointment

Please read our instructional in-take form to see if we are a match.  If you agree we are a match, please sign it (using your mouse or if on your phone, then a finger signature works)

Because intro and doctor appointments are limited, we will ask that you to prepay to hold your appointment.   

You will be 100% refunded if you cancel your appointment 3 business days before your appointment (We are open M-F and closed on major holidays)

You will receive helpful text reminders for your appointment!!!

We are located east side of Power Road, just north of Warner Road and across the street from the truBLISS dispensary.  We offer discounts if you are a truBLISS patient with proof.  Ask for details.  We are all about better alternatives in self-care!
